Submitting the Waiver Form (In-person or via Email):

Download the Waiver form from the given link.
Complete all the necessary fields with accurate information and ensure you have signed and dated the form.
If submitting in person, kindly hand over the completed waiver form to our staff at the designated location. If submitting via email, scan the filled form and send it as an attachment to FoldaWish@gmail.com.
